- Birth Certificate
- National Service Certificate
- SSNIT/GRA TIN Number
- Medical Report from Government Hospital Certified Medical Practioner
- Acceptance Letter
- Photo Copies of Two National ID Cards
- Flat Files
- Passport Sized Photograph
